Endodontics (from Greek endo- 'inside' and odont- 'tooth') is the dental specialty concerned with the study and treatment of the dental pulp.[1]

Overview

[edit]

Endodontics encompasses the study (practice) of the basic and clinical sciences of normal dental pulp, the etiology, diagnosis, prevention, and treatment of diseases and injuries of the dental pulp along with associated periradicular conditions.

In clinical terms, endodontics involves either preserving part, or all of the dental pulp in health, or removing all of the pulp in irreversible disease. This includes teeth with irreversibly inflamed and infected pulpal tissue. Not only does endodontics involve treatment when a dental pulp is present, but also includes preserving teeth which have failed to respond to non-surgical endodontic treatment, or for teeth that have developed new lesions, e.g., when root canal re-treatment is required, or periradicular surgery.[2]

Endodontic treatment is one of the most common procedures. If the dental pulp (containing nerves, arterioles, venules, lymphatic tissue, and fibrous tissue) becomes diseased or injured, endodontic treatment is required to save the tooth.[3]

Procedures

[edit]

Root canal treatment

[edit]

Root canal treatment is a dental procedure used to treat infected tooth pulp which would be otherwise extracted. The pulp is the soft tissue core of the tooth which contains nerves, blood supply and connective tissue necessary for tooth health.[4] This is usually caused when bacteria enter the pulp through a deep cavity or failed filling.[4]

Root canal treatment is required when the dental pulp is irreversibly damaged and involves both coronal and apical pulp.[5] Root canal treatment can also be carried out on teeth with doubtful pulpal state before placing post-retained crowns and overdentures.[6] Root canal therapy is not only performed when pain relief from an infected or inflamed pulp is required, it is also done to prevent adverse signs and symptoms from the surrounding sequelae and promote the healing and repair of the surrounding periradicular tissues. An example of this is if there is trauma to a front tooth which has caused it to be avulsed from the bony socket; endodontic treatment is required following re-implantation to preserve the aesthetics and function of the tooth, even though there may be no adverse symptoms of the dental pulp, or pain present at the time.[7][8]

Prior to root canal treatment, clinical examination and radiographic examinations are carried out to diagnose and plan treatment. Local anaesthesia is delivered to make the procedure pain free. The tooth to be treated is then isolated using a rubber dam, which prevents saliva entering the tooth during treatment and protects the airway from the fine files and strong chemicals used. The root canal treatment procedure can be carried out over single[4] or multiple appointments.[5] Root canal treatment involves:[9]

  • Removing the damaged and infected pulp
  • Shaping the entire root canal system
  • Cleaning and disinfecting the entire root canal system
  • Filling and sealing the root canal system
  • Placing a direct restoration such as composite filling or indirect restoration such as a crown

Instrument fractures are common procedural mishaps in root canal treatment. It is essential to prevent instrument fractures. The success of removing a broken instrument depends on the location, direction and type of instrument.[10] A sodium hypochlorite accident can result in long-term functional and aesthetic complications. Extrusion of sodium hypochlorite irrigating solution during a root canal procedure can cause a severe inflammatory reaction and tissue damage. Treatment is provided based on the severity of the injury.[11] Tooth discolouration as a result of root canal treatment can occur if the pulpal tissue remnants are not completely removed or if a root canal sealer material containing silver is used.[10]

Periradicular surgery

[edit]

Periradicular surgeries involve the root surface. These include apicoectomy (removal of a root end), root resection (removal of an entire root), repair of an injured root due to perforation or resorption, removal of broken fragments of the tooth or a filling material, and exploratory surgery to look for root fractures.[12][13]

Apicoectomy

[edit]

An apicoectomy is a surgical procedure through which the apex of a root is resected, and a root-end filling is placed, preventing bacterial leakage into the root canal system from the periradicular tissues.[14] A microsurgical technique is used to carry out apicoectomy, which improves post-operative healing.

An apicoectomy can be carried out when a previous root canal treatment fails, and re-root canal treatment is not possible.[14] This may be as a result of anatomical features, such as root dilaceration, which can compromise the completion of cleaning and obturating the root canal system. Procedural errors, including ledges or perforations, may also be indications for an apicectomy.

Local anaesthetic is utilised to achieve anaesthesia as well as haemostasis for improved visualisation. A flap in the gum is designed, and then raised to allow for exposure of the periapical lesion.[15] Bone removal (osteotomy) is carried out to enable access to root apex, and diseased tissue is removed at this point through curettage.[15] The root end resection is carried out, removing 3mm apically. The canal(s) is then obturated, and the flap is sutured. There are a number of root-end filling materials available, including zinc oxide eugenol cements, and mineral trioxide aggregate.

Complications that may arise include:

  • pain: anti-inflammatory agents or analgesics should be taken
  • swelling: intermittent ice will aid in eradicating this. Swelling resolves usually within 24–48 hours.
  • ecchymosis (discolouration): this will often occur distant from the surgical site
  • paraesthesia: usually transient as a result of inflammatory swelling, and sensation will return to normal in 4 weeks
  • serious infection is rare, but can be treated with antibiotics, which should be administered with caution to avoid bacterial resistance [16]
  • maxillary sinus perforation [17]

Other procedures

[edit]

Other non-surgical endodontic procedures include pulp capping, pulpotomy, apexification, and pulpal regeneration. Hemisection, where a root and its overlying portion of the crown are separated from the rest of the tooth and optionally removed, is another (non-periradicular) endodontic surgery.[13]

Tools

[edit]

Microsurgical endodontics, the use of magnification devices such as microscopes, and dental loupes, has been widely accepted among endodontists and practitioners;[18] its use is believed to increase accuracy and visualization in the operating field.[19] However, a Cochrane review in 2015 found no evidence to determine whether there is a difference in the outcome of a procedure done by magnification devices or a conventional procedure done with no magnification.[20] The American Association of Endodontists strongly encourages its members to pursue the use of an oral microscope to ensure the highest level of excellence.[21]

The use of a CBCT is also becoming the standard of care.[22][23]

Training

[edit]

Endodontists are specialist dentists with additional training, experience and formal qualifications in endodontic treatment, apicectomies, microsurgery, and dental emergency and trauma management.[18] Endodontics is recognized as a specialty by many national dental organizations including the Dental Board of Australia, British General Dental Council, American Dental Association, Royal College of Dentists of Canada, Indian Dental Association, and Royal Australasian College of Dental Surgeons.

Australia

[edit]

In Australia, endodontics is recognized as one of the thirteen registered dental specialties. In addition to a dental degree, endodontists have an additional three years of postgraduate University training in the area of endodontics to be recognized and registered by the Dental Board of Australia. A general dentist is permitted to perform endodontic treatment, but must be competent in the skills required for the treatment and refer complex cases for specialist management.[citation needed]

United States

[edit]

In the United States after finishing a dental degree, a dentist must undergo 2–3 additional years of postgraduate training to become an endodontist. American Dental Association (ADA) accredited programs are a minimum of two years in length. Following successful completion of this training, the dentist becomes eligible to sit for the American Board Of Endodontology examination. Successful completion of board certification results in Diplomate status in the American Board of Endodontics.[24]

Although general dentists can perform endodontic treatment, there are several things which set endodontists apart. Endodontists use microscopes during treatment to better treat the small internal anatomy of teeth without taking away too much tooth structure, or causing iatrogenic damage. Also, endodontists use CBCT (3D imaging) to assess case difficulty and for diagnosis and treatment planning of endodontic cases.

An oral emergency situation is a concern including the teeth and sustaining tissues that are of high importance to be dealt with by the relevant specialist. Dental emergencies do not always include discomfort, although this is an usual signal that something requires to be looked at. Discomfort can originate from the tooth, bordering tissues or can have the feeling of coming from the teeth but be brought on by an independent resource (orofacial discomfort and tooth pain). Depending upon the sort of discomfort experienced a skilled clinician can identify the most likely reason and can deal with the concern as each cells type provides various messages in a dental emergency. Many emergency situations exist and can vary from bacterial, fungal, or viral infections to a fractured tooth or dental remediation, each requiring a private action and treatment that is one-of-a-kind to the circumstance. Fractures (dental trauma) can happen anywhere on the tooth or to the bordering bone, relying on the site and level of the fracture the treatment choices will differ. Dental remediation falling out or fracturing can additionally be taken into consideration an oral emergency situation as these can influence feature in relation to visual appeals, eating and pronunciation and as such should be often tended to with the very same haste as loss of tooth cells. All oral emergencies should be treated under the supervision or advice of a dental health specialist in order to protect the teeth for as lengthy as possible. By contrast, a clinical emergency is often more exactly defined as an intense condition that provides an instant risk to life, limb, vision, or lasting health and wellness. Subsequently, dental emergency situations can hardly ever be referred to as clinical emergencies in these terms. Some define an oral emergency in regards to the individual's willingness to go to for emergency oral therapy at any moment summarily, mentioning that individuals that are fussy about when they are readily available for treatment are not real emergency situation cases.:   702   There are often different opinions between medical professionals and individuals regarding what comprises a dental emergency situation. E. g. an individual might suddenly shed a filling, crown, bridge, and so on and although they are completely pain-free, still have fantastic cosmetic worries regarding the appearance of their teeth and demand first aid on the basis of viewed social impairment.

.

Geriatric dental care is the shipment of dental care to older grownups including medical diagnosis, prevention, monitoring and therapy of issues connected with age associated conditions. The mouth is described as a mirror of overall health and wellness, enhancing that dental health and wellness is an integral component of general health. In the elderly population inadequate oral wellness has actually been thought about a threat aspect for basic health problems. Older adults are much more susceptible to dental problems or conditions because of an increase in persistent conditions and physical/mental impairments. Hence, the elderly form a distinct team in regards to arrangement of treatment.

.

Dentures (also referred to as incorrect teeth) are prosthetic devices constructed to replace missing teeth, supported by the surrounding soft and difficult cells of the mouth. Traditional dentures are removable (removable partial denture or complete denture). Nevertheless, there are several denture designs, a few of which rely upon bonding or gripping onto teeth or oral implants (dealt with prosthodontics). There are two main classifications of dentures, the distinction being whether they fit onto the mandibular arc or on the maxillary arch.

.

Dental hygiene is the method of maintaining one's mouth clean and free of illness and various other troubles (e. g. foul breath) by routine brushing of the teeth (oral health) and taking on great hygiene behaviors. It is very important that oral health be accomplished regularly to make it possible for prevention of oral illness and halitosis. The most common sorts of dental illness are dental caries (tooth cavities, dental caries) and gum tissue illness, consisting of gingivitis, and periodontitis. General guidelines for adults suggest cleaning at the very least two times a day with a fluoridated toothpaste: cleaning before going to sleep during the night and after breakfast in the early morning. Cleansing between the teeth is called interdental cleansing and is as crucial as tooth cleaning. This is due to the fact that a tooth brush can not get to between the teeth and for that reason just removes concerning 50% of plaque from the surface of the teeth. There are numerous tools available for interdental cleaning that include floss, tape and interdental brushes; it is up to each person to pick which device they like to make use of. In some cases white or straight teeth are associated with dental hygiene. However, a sanitary mouth can have stained teeth or uneven teeth. To improve the look of their teeth, people may use tooth bleaching treatments and orthodontics. The value of the role of the oral microbiome in oral wellness has actually been significantly recognized. Data from human oral microbiology research study shows that a commensal microflora can change to an opportunistic pathogenic plants through complicated changes in their setting. These changes are driven by the host instead of the microorganisms. Ancient proof of calcified dental plaque reveals marked shifts in the dental microbiome towards a disease-associated microbiome with cariogenic microorganisms ending up being leading during the Industrial Revolution. Streptococcus mutans is the most essential germs in causing decays. Modern oral microbiota are dramatically less varied than historic populaces. Caries (cavities), for example, have ended up being a major endemic disease, affecting 60-90% of schoolchildren in industrialized nations. In contrast, tooth decays and gum diseases were unusual in the pre-Neolithic era and in very early hominins.

.

Pediatric dental care (previously pedodontics in American English or paedodontics in Republic English) is the branch of dentistry handling youngsters from birth through teenage years. The specialty of pediatric dentistry is acknowledged by the American Dental Organization, Royal College of Dentists of Canada, and Royal Australasian University of Dental Surgeons. Pediatric (likewise paediatric or pæ& aelig; diatric )dentists advertise the oral health of kids along with act as instructional resources for moms and dads. It is advised by the American Academy of Pediatric Dental Care (AAPD) and the American Academy of Pediatrics (AAP) that a dental go to happens after the existence of the very first tooth or by a child's very first birthday celebration. The AAPD has said that it is necessary to develop an extensive and obtainable ongoing relationship between the dental expert and individual –-- referring to this as the client's "dental home". This is because early oral assessment aids in the discovery of the onset of tooth decay. Early discovery is essential to keep oral health and wellness, change aberrant behaviors, and deal with as required and as simply as feasible. Furthermore, parents are given a program of preventative home care (cleaning, flossing and fluorides), a decays run the risk of assessment, details on finger, thumb, and pacifier behaviors, and may consist of guidance on avoiding injuries to the mouth and teeth of youngsters, diet regimen therapy, and information on development and advancement.

.

Oral aides are members of the oral group. They sustain a dental operator (such as a dental professional or other treating oral accessory) in giving extra reliable oral treatment. Oral assistants are differentiated from various other teams of dental auxiliaries (such as dental specialists, oral hygienists and dental technicians) by differing training, duties and client ranges.

Reviews for The Briars Dental Centre

Richard Dalgleish

I have been visiting the Briars for many years and it is really good in every way. Whilst visiting a dentist is not usually considered a great place to be, The Briars has a way of making it much easier that you think it will be. The Two treatments I had today were with Richard and Meryl who were both were more than kind, helpful and clear in their explanations. Thank you both so much.

Changed dentists due to relocating to a new area and chose Briars not only from the online reviews but also the appearance of the clinic. From first view to going inside I was taken back by how professional it all looked and felt and the friendliness of the staff was warm and welcoming. I was in for my first consultation before any treatment was undertaken and a big thanks and special mention to Chelsea (Consultation Technician) for not only having a friendly and upbeat manner but explaining everything in such detail and with a calmness that I was instantly at ease and knew I had chosen the right place. For anyone who may be anxious about going to the dentist I can't recommend Briars highly enough.

I've used the Briars for years for myself and my three boys and always found everyone to be kind, patient and extremely knowledgeable. My middle son, Arthur, who's now 13, has always really disliked his teeth as they are quite small with gaps. Over time our dentist, Afif, made a plan with him to add some composite bonding to his front four teeth. This meant Arthur had minimal intervention and preserved the integrity of his teeth, which was super important to me as he's still only young and growing, but gave Arthur the changes he needed to give him more confidence in his smile. We're completely overjoyed with the result, it's made the world of difference to how Arthur feels about himself and he's really pleased he had it done. The composite bonding was really reasonably priced and gives us the options of doing more as he grows and becomes an adult if we wish. Afif has been great throughout, talking both me and Arthur through his plan so we're both reassured about the process. I also wanted to mention how great Meryl is, she is the hygienist me and my three boys all use. My youngest son is quite nervous about the dentist and finds it tricky but she is so kind and caring with him and taken the time to build up trust over a number of appointments that now he feels a lot easier with her. I'd not hesitate to recommend the Briars to anyone.

I have been attending The Briars Dental Centre for a couple of years and cannot praise their expertise enough. Yesterday Dr. Lilleker prepared a back tooth for a crown. Although the treatment was lengthy, at no time did I feel hurried and had complete confidence in his professionalism, ability and care. Furthermore, the premises are laid out very well. Parking in my experience, has never been a problem, first class cleaners internally cannot be faulted, the reception staff always seem pleased to see patients and listen to them and the external grounds are always tidy. What more could anyone want?

My children love visiting the centre to see Dr Hanif. His care and passion is exceptional. Not only does he provide an educational and positive experience, but he builds a comfortable and relaxing environment. My children feel at ease throughout and he has a wonderful way of engaging patients at the right level to ensure the appointment is effective and enjoyable. As a parent, I could not ask for more. I'm included throughout the appointment and he has provided a great foundation for oral care through his approach and advice, which has the children very excited and motivated to continue. This level of professionalism and care extends throughout Briars. My family have always felt welcomed by this very knowledgeable and engaging team.
